Provides therapy treatments to PruittHealth patients in their place of residence.Essential Skill/Knowledge FunctionsResponsible for assisting the development and implementation of the plan of care within goals defined by the physician, patient/patient family and interdisciplinary team.Provides therapy services according to the plan of care that has been developed and ordered by the physician.Regularly reevaluates the patient’s level of function and skilled therapy needs.Prepares clinical documentation including assessments, visit and progress notes and coordinates services as directed by the RN Care Manager and or Patient Care Coordinator. Submits all documentation within timelines determined by the agency.Follows all company policies and procedures, state and federal laws and regulation and charged with reporting violations to the appropriate supervisor.Carries out all duties with a “Commitment to Caring” and in accordance with the company’s mission, vision and philosophy.Maintains confidentiality of company information.Demonstrates honesty and integrity at all times.Able to respond to change productively and handle additional projects as assigned.Able to carry out the duties and responsibilities of this job (with or without reasonable accommodation) without posing specific or current risk of substantial harm to the health and safety of self and others.Meets agency productivity standards and utilizes time and resources effectively.Instructs the patient/patient caregiver(s) on home care techniques, safety, etc. as indicated.Instructs the patient/patient caregiver(s) on the total Occupational therapy program.Advises and consults the family and other interdisciplinary team members to promote the optimal functional outcomes.Performs other duties as assigned by Patient Care Coordinator.Essential Administrative FunctionsAssist, if needed, in center/agency surveys (Licensure / JCAHO) and any subsequently required reports.Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.Attend and participate in mandatory in-services.Honor patients/residents’ rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.Comply with corporate compliance program.Report job-related functions/tasks that involve occupational hazards.Follow established safety regulations, to include fire protection & prevention, smoking regulations, infection control, etc.Follow established safety procedures when performing tasks and/or working with equipment.
