SC MENTOR: Founded in 1984, South Carolina MENTOR provides home and community- based services to children and adults with intellectual and developmental disabilities as well as youth facing emotional, behavioral and medically complex challenges. South Carolina MENTOR offers an array of cost-effective residential treatment options including Therapeutic Foster Care and Host Home programs, as well as coordinating support service for children and adults throughout the state.

SC MENTOR isseeking a Clinical Coordinator to provides case management and family support services forchildren, youth, and treatment foster parents supported by the program; Conductdiagnostic assessments as needed for children/youth referred for services; Workcollaboratively with the child/youth and treatment team in the development ofindividualized plan of care; Creates, coordinates and evaluates services plansthrough partnerships with stakeholders based on assessed client needs; Providespre-service introduction and in-service continuing education for treatmentfoster parents as appropriate per regulatory requirements. Provides ongoingmonitoring and consultation to treatment foster parents regarding work withchildren and youth supported and implementation of service plan; conducts homevisits; facilitates support groups; Maintains quality documentation inaccordance with corporate and regulatory standards; participates in rotationalon-call schedule; perform other assigned duties as needed.
